CHF: exertional dyspnea, JVD, peripheral edema, elevated BNP, can be chronic or acute decompensated

  • exertional dyspnea, JVD, peripheral edema, elevated BNP, can be chronic or acute decompensated
  • Pulm/left (SOB, orthopnea, PND), right (edema, nocturia), hx of heart dz, smoke, hypertension, obesity, diabetes, valve dz
  • hypoperfusion, Pulm/left (lung sounds, P2↑), right failure (JVD, edema), extra heart sounds/gallops, pulse pressure↓
  • BNP↑, NT pro-BNP↑, cardiomegaly
  • left = blood backups up into lungs, pulm HT. Right = blood backs into periphery, edema. Cold = hypoperfusion, cool extremities. Wet = pulm edema.
  • diuretic, ACE inh, ARB, beta blk, aldosterone antag, nitrates, digoxin (if low EF). Inotropes if cold extremities. Diuretics if pulm edema.
