  • SOB, smoking hx
  • fast, shallow breathing, barrel chest, pink puffer
  • CXR: less lung tissue (centrolobular/upper lobe if smoking, panlobular/lower lobe if a1 antitrypsin↓), flat diaphragm. FEV1/FVC↓, (TLC, FRC, RV)↑, DCLO↓
  • destruction of alveola septa (elastase) = less surface area. Due to smoking (upper lobe, centrilobular), alpha 1 trypsin deficient (lower lobe, panlobular), homozygouse ZZ (liver can't secret alpha1 antitrypsin)
  • quit smoking, lung vol reduction surgery
