  • Low blood sodium (Na < 136) causes cerebral edema leading to neuro problems (confusion, seizure, coma)
  • Headache, nausea, altered mental status, then seizures, comas, respiratory arrest
  • Neuro: confusion, gait
  • Na < 136
  • Cerebral edema (water leaks into brain cells, osmotic) followed by cerebral solute loss (solutes leak out of swollen brain cells, compensatory)
  • Fluid restriction if not volume depleted (edema, SIADH), isotonic saline if volume depleted (diarrhea, sweat), hypertonic saline (SIADH), loop diuretic (SIADH)
  • SIADH = syndrome of inappropriate ADH
