RPGN: nephritic (hematuria, red casts) with severe proteinuria and kidney failure (GFR--, oligouria) due to crescent scars (leaked fibrin -> inflammation) in glomeruli

  • aka: Rapidly progressive glomerulonephritis, crescentic glomerulonephritis,
  • nephritic (hematuria, red casts) with severe proteinuria and kidney failure (GFR--, oligouria) due to crescent scars (leaked fibrin -> inflammation) in glomeruli
  • fatigue, edema, dark urine
  • edema, hypertension
  • proteinuria (albumin↓), hematuria (red casts), oligouria, GFR↓ (rapid, severe), glomerular crescents, immuno (positive for anti-GBM, immunocomplex, but not for pauci-immune)
  • Anti-GBM (Goodpasture), Immune complex (Henoch, IgA), Pauci-immune (Wegener, Churg). Crescent = scar formed by proliferation/inflammation caused by leaked fibrin
  • Lots of crescents that are circumferential = bad prognosis.
