SIADH: Hyponatremia with natriuresis (urine Na > 40), no edema or dehydration

  • aka: Syndrome of inappropriate ADH,
  • Hyponatremia with natriuresis (urine Na > 40), no edema or dehydration
  • suspicious Hx (brain tumor/trauma, small cell lung car, drugs)
  • neuro (confusion, gait)
  • Na↓, ADH↑, urine Na > 40
  • water retention (ADH) dilutes blood sodium while natriuresis prevents edema. Water retention = no need for aldosterone = natriuresis.
  • Fluid restriction, hypertonic Na (if severe), V2 inhibitors (coni/tolvaptan)
  • V2 drugs are Vaptans
